“What is Palliative Care Really?  Enhancing the social Worker’s Understanding of Palliative and Hospice Care”

Increasing population challenges in chronic illness welcomes palliative care as an answer to improving the quality and quantity of life.  Early introduction of palliative care eases the transition to hospice care and the social worker’s role is often key to a successful outcome.  This presentation will discuss the differences between Palliative Care and Hospice Care, the decision-making process in advanced illness, and how a team approach works to ensure the most positive outcome in this journey.

Dr. Joseph Sacco, CMO, The Connecticut Hospice, Inc., will be the presenter.

This presentation has been approved for 1.5 CEUs.
