THE POSITION
We are seeking a qualified individual for the position of Clinical Social Worker Associate to join the CRMHC Mobile Crisis Unit. The position number is 26998. The reporting location of the program is 500 Vine Street in Hartford. This position is full-time, 40 hours per week, second shift, and has a two-week, rotating work schedule from 4:00pm to 12:30am, including every other weekend and holidays. The schedule rotation is as follows:

Week 1: Working on Monday through Friday
Week 2: Working on Saturday, Sunday, Monday, Wednesday, Thursday

The Mobile Crisis Unit provides psychiatric crisis services. These services include evaluations of individuals in psychiatric crisis at the Center and in the community (individuals' homes, jails, shelters, residential programs, hospital emergency rooms, nursing homes, and in other mental health facilities.) The Mobile Crisis Team assesses the individuals' need for services, and assists in resolving the crisis. In addition, assessment services are offered during normal working hours to assist clients in obtaining services at Capitol Region Mental Health Center or to refer clients for services to other area providers.

stakeholders to assist in developing transition plans for identified youth;
-Assist with planning around preparing youth for transition to DMHAS YAS, participate in interagency planning meetings and case conferences, and assist with orienting youth, family members and other stakeholders to YAS;
