THE POSITION
We are currently recruiting for two Clinical Social Worker Associates to work within our Torrington Access Center (Mobile Crisis Team). These roles will be located at 249 Winsted Road in Torrington. Our team is passionate about the work we do for our clients! These are third shift, full time, forty (40) hour per week roles. The positions will have the following work schedules:

PCN 130484: Sunday through Thursday, from 11:45 p.m. to 8:15 a.m.
PCN 130485: Tuesday through Saturday, from 11:45 p.m. to 8:15 a.m.
This position functions as a member of an interdisciplinary Mobile Crisis Team that provides crisis evaluations and interventions in the community. The incumbent will demonstrate the ability to clinically assess risk in the community, to formulate immediate interventions, and administration of Emergency Certificates, as clinically indicated. The goal is to provide support and information to reduce symptomatology, and to improve quality of life. Its philosophy is to maintain the individual in the least restrictive environment. Access Center staff is also available to do intake/assessment to determine service needs and treatment options for an individual.

DISCOVER THE OPPORTUNITY TO:
Provide services via the Torrington Hotline with the skills to access/triage/activate resources for people who request information, support or crisis intervention.
Provide intake services to the 19 towns in the CAC 22 area. This is done by completing a psychosocial and risk assessments, and other admission paperwork for admitting clients to the agency.
Communicate and work collaboratively with contract agencies, community providers, other State of CT DMHAS offices and community hospitals as well as local and State police. You will work to link community members to appropriate care, provide community resource information and manage risk for negative outcomes in the community.
Provide early behavioral health intervention to community members, brief treatment in individual and/or group modalities, and education and training to community.
Provide coverage for the Jail Diversion program as assigned by the program supervisor. This includes completing assessments, and making clinical recommendations for treatment as requested by the court. You would coordinate care and monitor participation, with psychiatric and substance abuse treatment services.

This position is part of a multi-disciplinary team responsible for the intake and processing of clients who are new to the SWCMHS. DEC/COES is the initial point of entry into the SWCMHS, manages admission to all SWCMHS programs and is designed to quickly identify an individual’s needs and refer them to the appropriate level of care either within SWCMHS or in the community.

DEC/COES provides a full range of assessments to determine an individual’s needs and makes recommendations for appropriate follow-up care. The team provides short-term treatment services while a referral is processed to either a treatment team within SWCMHS or to a community provider. The Center’s primary functions include: clinical evaluation, psychiatric evaluation, psychological evaluation, health evaluation, TBI/ABI screening, employment/education screening, psychosocial assessment, brief treatment services, referral to clinical and primary health services.

We, at the Connecticut Mental Health Center, are currently recruiting for two Clinical Social Workers (PCNs 26731, 26535), to work with our Hispanic Clinic. Our clinic is located at 34 Park Street in New Haven. These are full time, 35 hour per week roles. The social workers will be on first shift, with a work schedule of 9:00 a.m. to 4:30 p.m., Monday through Friday.

Due to the nature of this position our ideal candidate will be bi-lingual in both English and Spanish.

Since its inception in 1973, the Hispanic Clinic has served the mental health and addiction needs of the monolingual Latino community of the greater New Haven area. The Clinic staff is bilingual/bicultural and representative of the diversity of the Latino cultures. Clients are self referred or referred by a variety of health and human service agencies in the community.

YAS provides a comprehensive program of treatment to clients 18 to 25 years of age with severe and persistent psychiatric and substance use problems.
