The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS), Office of the Commissioner (OOC), is seeking a qualified individual for the position of Clinical Social Worker Associate (35 Hour) within the Young Adult Services Division.

DMHAS Young Adult Services (YAS) was developed to help young adults transition successfully from the Department of Children & Families (DCF) to the adult mental health system and to achieve the necessary skills for adulthood. The division services individuals between the ages of 18 and 25 who often have a history of DCF involvement and major mental health challenges. The division's primary goal is to improve the lives of young people by providing the highest quality services possible. To achieve this goal, DMHAS YAS focuses on forming a partnership with the individual, their family, identified significant persons, and with other community service providers. Creating a "community of care" fosters mutual respect and individualized client centered treatment.

POSITION HIGHLIGHTS
This OOC YAS position will be located on the Connecticut Valley Hospital (CVH) campus at 1000 Silver Street, Middletown, CT. This is a full time, first shift, thirty five (35) hour per week position, with a work schedule of Monday through Friday, 8:30 a.m. - 4:00 p.m. This is a hybrid position that features telework opportunities in accordance with State policies. In this role, discover the opportunity to:
-Provide transitioning direct care services to psychiatrically complex, high risk youth and their families who are referred to DMHAS YAS from DCF, Court
Support Services Division (CSSD) and Beacon Health Options;
-Obtain and review referral information and clinical documentation, complete eligibility determinations, conduct clinical interviews and collaborate with
stakeholders to assist in developing transition plans for identified youth;
-Assist with planning around preparing youth for transition to DMHAS YAS, participate in interagency planning meetings and case conferences, and assist with orienting youth, family members and other stakeholders to YAS;
-Collaborate with DCF, Beacon Health Options, CSSD and OOC YAS program staff to ensure that all elements of a youth’s transition plan are in place prior to the young adult’s admission to a YAS program;
-Input, track and manage eligibility-related data in accordance with the DCF, Beacon Health Options, and CSSD Memorandums of Agreement (MOA).
