DISCOVER THE OPPORTUNITY TO:
Be a part of a multi-disciplinary team and the agency's largest outpatient clinical team;
Provide clinical services to adults with moderate to severe mental health and substance use disorders;
Independently develop comprehensive psychosocial assessments through interviews with assigned adult clients, their families and community providers;
Provide individual and group treatment, develop crisis response plans and provide case management as needed;
Develop recovery-oriented treatment plans that include measurable treatment goals and objectives;
Monitor and evaluate the effectiveness of treatment plans;
Consult and collaborate with other behavioral health and community service providers to improve and coordinate delivery of services to clients;
Document treatment in clinical records, and will complete clinical reports in compliance with agency, billing, and accrediting bodies; and
Participate in agency committees.

CMHC is an urban community mental health center with an exceptional 50-year tradition of serving individuals with serious mental illness and/or addiction. Founded in 1966, CMHC is a partnership between DHMAS and the Yale Department of Psychiatry. CMHC is nationally recognized for its scientific research into the causes and treatment of mental illness and addiction. The Abraham Ribicoff Research Facilities include multiple research programs in a variety of areas. CMHC is also a major training site for individuals pursuing careers in mental health fields including psychiatry, psychology, social work, nursing and pastoral care.
