Many of the individuals who occupy the unit’s 20 beds are already in treatment at the Center or have a prior history of inpatient care at CMHC.

Psychiatric, social work, nursing and rehabilitation assessments guide the treatment, which is provided on the unit by a multi-disciplinary team, under the direction of an attending psychiatrist, and in collaboration with any providers who are involved in an individual’s outpatient care. Treatment involves pharmacotherapy, individual, group, family, occupational, rehabilitative and milieu therapy, as well as medical treatment and assistance with discharge planning, including housing, finances, social and vocational rehabilitation, and the arranging of ongoing medical and psychiatric treatment. Length of stay on the unit varies depending upon the time needed by each individual for thorough assessment, stabilization, and development of a comprehensive discharge plan.

Complete psychosocial assessments, risk assessments and other intake documents;
Develop comprehensive recovery-oriented, person-centered treatment plans including measurable objectives;
Document client contacts and treatment in accordance with agency policies, and coordinate services including referrals to other community providers; and

AS A CLINICAL SOCIAL WORKER YOU WILL:
Provide team leadership and supervision to staff within the MOT program;
Provide crisis assessments and interventions including risk management;
Conduct intakes for referral to treatment at SMHA;
Provide clinical/crisis interventions and consultations to clients and staff;

MOT provides a wide range of crisis services: 24-hour crisis assessment and intervention services for people experiencing mental health and/or substance abuse challenges; hospital and crisis bed screening; referrals for temporary respite; support phone calls and visits; brief crisis counseling; and referrals to appropriate inpatient and outpatient services in the community. The Mobile Outreach Team is able to respond in the community from 8:00am to midnight daily to assist individuals, families, and community agencies when an individual is experiencing a psychiatric and/or substance abuse crisis.
