THE POSITION
We are seeking a Clinical Social Worker to help support our agency.

Facility: Connecticut Mental Health Center (CMHC)
Program: Community Support Program
Location: 34 Park Street, New Haven, CT 06457
PCN: 26263
Schedule: Full time, 40 hours per week | First shift, 8:30am to 5:00pm

This position is assigned to CMHC’s Community Support Program, providing services to adults with moderate to severe mental health and substance use disorders. The CSW leads designated staff to provide integrated clinical, case management and skill building services to outpatient clients who require these supports to maintain independent living in the community. The CSW works as a member of a multidisciplinary treatment team in office and in the community.

AS A CLINICAL SOCIAL WORKER, DISCOVER THE OPPORTUNITY TO:
Provide individual and group treatment and develop person-centered recovery plans, involving family and other community providers as clinically indicated and agreed upon by the individual in recovery.
Develop crisis response plans, provide crisis assessments and have or complete training required to write LCSW transportation certificates to facilitate hospital evaluations.
Document treatment in clinical records, complete reports in compliance with agency, billing and accrediting bodies and adhere to all CSP fidelity standards, as established by DMHAS.
May participate in agency committees and perform related duties as assigned.
