THE POSITION
The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is seeking an experienced Clinical Social Worker to join their team. The successful candidate will have experience in the provision of adult outpatient behavioral health services, in Spanish.

DISCOVER THE OPPORTUNITY TO
Care for both English-speaking and mono-lingual Hispanic adults with cultural competence;
Conduct telephone and walk-in screenings to determine eligibility for services and/or service needs;
Utilize interviewing and assessment skills to gather developmental, social, clinical, and legal history for the purpose of evaluation, treatment, and disposition planning for assigned clients;
Provide case management services and provide ongoing individual, family and/or group treatment;
Develop knowledge regarding the types of substance abuse services and resources available in the community and assist clients to access and utilize needed services and resources to facilitate treatment and discharge planning;
Develop, monitor and evaluate the effectiveness of recovery-oriented, client-centered treatment plans;
Attend all clinical rounds and treatment planning meetings and routinely review work with a supervisor and/or other appropriate staff to ensure quality of care; and
Ensure that all clinical documentation is completed in a thorough and timely manner, in accordance with CMHC guidelines and the guidelines.
