As a Care Manager you will coordinate services and provide support and guidance to your clients, enabling them to remain successfully in the community, avoiding unnecessary hospitalization or institutional placement. Your responsibilities will include: Performing assessments of the client's health, functional, and cognitive status. Empower the client and/or client supporters to participate in the development of a plan of care. Educate the client regarding available resources and arrange for or refer to appropriate service providers. Advocate and facilitate for the client as needed to maintain maximum independence. Assist with any required applications for supportive services or programs. Maintain documentation of activities and client contact and prepare required reports. Meet critical deadlines established by funding sources and internal procedures. Monitor effectiveness of plan or care, and work with the client to adjust as needed.

We are looking for a candidate who is excited by an opportunity to work, play, teach, and supervise in a peaceful long-term therapeutic and educational agency serving predominantly adolescent girls and young women in private school, farm, rural, and family style environments.
The clients and students who come to Wellspring and the Arch Bridge School typically present with issues that stem from, but are not limited to mood disorders, anxiety disorders, trauma, victimization, suicidal behaviors, personality disorders, ego dystonic eating disorders, family problems, adoption issues, substance abuse in partial remission, and school avoidance. Interest and experience with these issues, psychodynamic theories, trauma informed care, and family systems is desired.
A minimum of ten years clinical experience in gradually increasing leadership roles is preferred. Teaching, mentoring and public speaking experience is a plus. MD, Ph.D., Psy.D., PMH-APRN, LCSW, or LMFT required.
