Care Managers provide highest standard of care management to clients wishing to remain safely in their communities/homes.
Conduct assessments and reassessments of clients as assigned.
Maintain an accurate, current roster of clients.
Coordinate home visits to clients at least every six months.
Complete a 30-day update on clients as assigned.
Participate in the process of assessing the discharge of clients from a hospital or nursing home in order to coordinate services.Develop client care plans.

Housing Coordinators work with identified Money Follows the Person clients to provide housing coordination services.

Identify potential units, including but not limited to assisted living units, apartments, individual homes, and subsidized housing, based upon the results of the client’s needs assessment and which meet the criteria of the MFP operating protocol.
Show and/or provide each client with at least two housing listings, with the exception of clients who have found housing earlier in the process.
Assist clients with arranging transportation to view units, and provide the clients with contact information for public or private transportation entities available for transporting the participant from the nursing home to the unit.
Inform all clients that the clients are responsible for final transportation arrangements.
Make funds available to the client, within the limitations established in the budget, for the payment of transportation if the client’s funds are not sufficient to cover the cost. Contact the transportation entity on behalf of clients with communication disorders.
Work with Transition Coordinators in assessing housing needs of clients including identifying need for modifications to a rental unit, based on the Department approved accessibility check list.
Identify and gain landlord approval regarding accessibility modifications required to make housing accessible to clients.
EDUCATION Bachelor’s degree from an accredited four year college or university.Experience and or knowledge of housing resources and landlord/tenant CT regulations.
